They both have the Linux Kernel http://www.kernel.org/
The Kernel is the main "BRAIN" of the operating system.
There are thousands of linux distributions ( you can see most of them here: http://distrowatch.com/ )
The main difference in them, are the packages (software) included.
The other main difference is the Package Manager and X-Windows system is uses (Usually it’s either KDE or GNOME).
So if a 700MB distro would have many packages already included (like gAIM, xchat.. etc.) while others are minimal, and let you install the packages / apps you want.
It’s all a matter of taste!
As netsploit said the kernel is the same but some people want to or have to use a smaller OS because they have limited ram and hard drive space or an older cpu. The smmaller ones use a lighter desktop instead of kde or gnome, and do not include very many programs on the install disk. THere are other who use the smaller OS and have plenty of ram and cpu power but want a system that is fast and more responsive. They can later install all the programs that the other larger OS has.
The larger OS also has a lot of eye candy that some people do not want.
